Responsibilities

• Perform initial PT evaluation and develop an individualized plan of care for home health patients within the care framework.
• Implement therapeutic interventions including therapeutic exercise, gait and balance training, ROM, strength training, and functional mobility training.
• Educate patients and families on home exercise programs, safety, and strategies to support daily activities and functional independence.
• Monitor and document patient progress, adjust treatment plans, and communicate updates to the supervising clinician and care team.
• Collaborate with interdisciplinary team to coordinate care, schedule visits, and support discharge planning and community resources.
• Ensure accurate, timely documentation in the patient record and maintain compliance with applicable standards and regulations.

Requirements

• Licensure: Active physical therapy license in the state of practice (California)
• Certifications: CPR certification or Basic Life Support (BLS or equivalent) required
• Experience: Minimum 2 years of physical therapy practice
• Specialty: Focused experience in Home Health or equivalent care setting
• Ability to work in a home health care delivery model and travel as assigned
• Strong documentation, communication, and patient education skills
