Responsibilities

• Assess and treat patients in an outpatient/home health context with a focus on functional outcomes and discharge planning
• Develop, implement, and adjust individualized physical therapy plans of care based on evaluation findings and progress
• Reassess patients regularly, document progress, and communicate results with the interdisciplinary team
• Educate patients and caregivers on therapy plans, safety, and home exercise programs; ensure appropriate referrals when needed
• Maintain accurate documentation in the medical record and comply with licensure, safety, and facility policies
• Collaborate with nursing, rehabilitation staff, and case management to support timely patient discharge and transition of care

Requirements

• Active Physical Therapy license in the state of practice (California)
• BLS certification (Required) - 1 year of physical therapy experience; 2 years preferred
• Must have worked in the unit/therapy specialty within the last 3 months
• Ability to work in an outpatient/home health setting within a hospital-type environment
