Requires an Associate degree in nursing and a current Wisconsin Registered Nurse license. Minimum of 2 years of experience providing care for frail or elderly populations is required, with recent case management experience preferred.
RN Care/Case Manager, Full-Time
Community Care is offering a $8,000 Sign-on Bonus for Full-Time RN Care/Case Manager to join the Day Shift. This is a full-time opportunity for Rock County, based out of our Walworth office. Hybrid work is available.
· Collaborate with the site management team to establish and maintain uniform, consistent care delivery for Community Care members, including collaborative problem solving related to member appointments, transportation, and home care needs. · Conduct phone and in-person assessments using decision tools and resources to identify immediate and potential health needs · Complete all required documentation and charting and assists in the development and maintenance of member records
· 8:00 a.m. - 4:30 p.m., 40 hours a week
