Provide longitudinal, holistic primary care to homebound seniors and medically complex patients within their homes and communities. Manage chronic and acute conditions while collaborating with an interdisciplinary team to reduce hospital readmissions.
Must be a Nurse Practitioner or Physician Assistant with a master's degree and active licensure. Requires a passion for serving senior populations and the ability to adapt to home-based clinical environments.
