Conduct comprehensive hospice admission assessments in home and hospital settings while educating families on hospice philosophy. Coordinate seamless transitions from hospital to home, including DME ordering and medication reconciliation.
Requires a current North Carolina RN license, a valid driver's license, and at least 2 years of nursing experience, preferably in hospice or home health. Candidates must possess strong clinical assessment skills and proficiency in EMR documentation.

Full-time - 36 or 40 hours per week; Weekdays with occasional weekend rotation - various schedule options available for discussion
