Responsibilities
Provide compassionate direct and indirect nursing care to homecare patients and families throughout the service area
Deliver nursing services including assessments, symptom management, preventative care, education, and time-of-death care according to the plan of care
Work collaboratively across homecare teams to provide daily visits in patient homes, long-term care facilities, and assisted living facilities
Complete assigned visits per day or as determined by clinical leadership based on patient acuity
Communicate appropriately and promptly with physicians and other clinicians to support patient and family needs
Maintain accurate and timely clinical documentation and physician orders within the Electronic Medical Records (EMR) system
Qualifications
Current North Carolina Registered Nurse (RN) license required
Strong nursing assessment and critical thinking skills
Excellent organizational and problem-solving abilities
Exceptional customer service and communication skills
Ability to foster a positive culture built on respect, compassion, and teamwork
Experience with electronic medical records required; Axxess EMR experience preferred
