About the role

The pharmacist provides evidence-based pharmaceutical care by designing and monitoring therapeutic plans for patients in an ambulatory setting. They collaborate with interdisciplinary teams to manage drug therapy and improve patient health outcomes.

UVA Physicians Group is seeking an Ambulatory Care Clinical Pharmacist for two of our primary care practice locations, UVA Primary Care Riverside in Charlottesville, VA and Medical Associates of Louisa in Louisa, VA.

The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting.

The delivery of care by the Ambulatory Care Clinical Pharmacist includes, but is not limited to: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams in the ambulatory clinical setting.

When appropriate, pharmacists who provide ambulatory care services may perform patient assessments, participate in collaborative practice agreements to provide medication management, provide collaborative drug therapy management, order, interpret, monitor medication therapy-related tests, coordinate care and other health services for wellness and prevention of disease, provide education to patients and caregivers, and document care plans in the medical record.

Supports and enhances the quality of care in ambulatory practice areas.

Enhances patient access to affordable care.

Supports the educational and professional development of healthcare professionals.

Engages in professional self-development.

In addition to the above job responsibilities, other duties may be assigned.

Job Description

MINIMUM REQUIREMENTS Education: Bachelor’s in Pharmacy with experience or Pharm.D. from an ACPE accredited College of Pharmacy. Degree required.

Experience

  • PGY-1 residency required or equivalent clinical experience.
  • License/Certification: Commonwealth of Virginia State Board of Pharmacy License required. American Heart Association (AHA) Health Care Provider BLS certification required. Board Certification or equivalent certification specific to clinical area highly encouraged. PHYSICAL DEMANDS Job requires sitting for prolonged periods;
  • Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull < 20lbs. May be exposed to fumes, chemicals, blood/body fluids and infectious disease. The University of Virginia is an equal opportunity employer. Requirements

Candidates must hold a Pharm.D. or Bachelor's in Pharmacy and possess a valid Commonwealth of Virginia State Board of Pharmacy License. A PGY-1 residency or equivalent clinical experience and BLS certification are required.
