The Patient Coordinator facilitates communication between healthcare teams and patients while providing essential support and resources. Key duties include accompanying patients to benefit renewal appointments and conducting home visits to update care plans.
Candidates should have 1-3 years of experience in a care team or patient navigation role and a high school diploma, though an associate degree is preferred. Dependable transportation is required for local travel between patient homes and offices.

The Patient Coordinator is responsible for enhancing the overall patient experience and ensuring that patients receive the necessary support and resources. Accompany patients to their benefit renewal appointments to advocate for the patient and the program. Visit patient homes and update patient care. Effectively communicate and provide information to patients, caregivers, and physicians. Understand HIPAA and maintain confidentiality. Display a warm and friendly attitude.
Strong work ethic and relationship building skills. Self-motivated with the ability to manage time appropriately. Strong communication skills inside of the office and outside. Experience with MS Office products (Outlook, Word, Excel, etc.) Minimum associate level education preferred. (High school diploma or equivalent is required.) Minimum 1-3 year of experience in a Care Team or Patient Navigation role. Dependable transportation to be able to travel locally between locations including patient homes, physician offices and Giving offices. Healthcare experience preferred.
