The Family Care Manager assesses the psychosocial needs of child and adolescent psychiatric patients and coordinates their care within the Ohana system. They serve as a central navigator for families, managing transitions between treatment levels and facilitating community-based referrals.
A Master's degree in Social Work and Associate registration with the BBS are required. Candidates must have at least 3 years of experience working with youth and families, along with BLS certification.

The Family Care Manager (FCM) reports to the Assistant Director of Outpatient and is responsible for assessing the social and psychosocial needs of child and adolescent patients and their families who are experiencing complex psychiatric challenges. The FCM plays a key role in coordinating care and managing transitions within the Ohana system of care, including admissions, discharges, and movement between levels of treatment. Serving as a central point of navigation for families, the FCM helps patients and caregivers understand and address factors that may impact behavioral health, including legal, educational, financial, occupational, transportation, and insurance-related concerns. The FCM also facilitates referrals to appropriate community-based services and programs outside of Ohana when needed and maintains knowledge of effective local and regional resources to support families. In this role, the FCM builds collaborative relationships with external providers and organizations while also providing psychoeducation through individual support, groups, and workshops. As part of a multidisciplinary mental health team, the FCM collaborates closely with therapists, psychiatrists, and other behavioral health professionals by participating in clinical meetings and advocating for the needs, strengths, and barriers of the family system from a social work perspective. The FCM also performs additional duties as assigned to support high-quality patient care. The Family Care Manager job family consists of two levels of professional, non-management roles: Unlicensed Family Care Manager and Licensed Family Care Manager.
Must have a minimum of 3 years’ experience working with children, adolescents or families and knowledge of the local and regional resources that are critical to the families of youth with behavioral health conditions, such as the educational system, legal resources, juvenile justice system, community services, and public insurance (MediCal) programs.
