Linkage to care specialist/ Total Healthcare

A Linkage to Care Specialist at Total Health Care facilitates access to medical and supportive services for vulnerable populations, particularly individuals newly diagnosed with or at high risk for HIV/AIDS and Hepatitis C. They focus on patient navigation, testing, and overcoming barriers to care Perform comprehensive assessments to identify psychosocial and medical needs. Develop individualized intervention plans using proven behavioral models like Motivational Interviewing and Harm Reduction. Attend community events, health fairs, and conduct targeted outreach to engage hard-to-reach or homeless populations.

Qualifications

Education: A High School diploma or GED is required; an Associate’s or Bachelor’s degree in Public Health, Social Work, or a related field is highly preferred.

Certifications

Maryland certification in Rapid HIV Counseling & Testing is typically required. Phlebotomy certification and Rapid Hep C training are often preferred.

Experience

Experience working within the HIV/AIDS community, harm reduction, or social service agencies in Baltimore.
