Requirements summary
Requires a valid Michigan Driver's License and Basic Life Support (BLS) certification. A high school diploma and previous caregiving experience are preferred.
Job Summary Provides services in the home or facility which include any of the following: personal care for an adult including hygiene and bathing, memory care, simple self-care procedures, vital sign and other health monitoring, use of assistive devices including wheelchairs, meal planning and preparation, supervision, transportation, companionship, medication reminders, light housekeeping and assistance with procedures as assigned.
Documents services on provided forms and systems.
Essential Functions Complies with standards to provide excellent customer service.
Adapts interactions in consideration of age-related, functional, cultural, developmental and psychosocial factors.
Completes all assigned tasks per established procedures.
Staff will need to provide medication reminders to clients as outlined in the care plan.
Reports safety issues and implements safety precautions.
Conducts required safety assessments and reports to office.
Demonstrates accountability for ongoing, professional and respectful communication.
Promptly notifies office of changes in client condition, team concerns & service issues.
Promptly complies with all employment requirements, program policies and procedures.
Submits timely and proper documentation.
Reports/appropriately handles incidents.
Demonstrates consistent reliability and responsibility in meeting client service assignments.
Demonstrates good attendance and promptness.
Uses corporate e-mail consistently and returns calls promptly.
