The Social Services Assistant coordinates patient support and discharge planning under the supervision of the Social Services Director. Key duties include conducting initial assessments and providing information on financial assistance programs like Medicare and Medicaid.

Wage Commensuration The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.
Complete the initial Social Service Assessment and the initial patient interview profile to begin discharge planning upon admission Provide information to family and/or patients/residents relating to Medicare, Medicaid, and other financial assistance programs available to the patient/resident Work closely with Case Management to coordinate patient discharges Qualifications & Requirements Must possess at minimum a High School Diploma or equivalent G.E.D Social Work License preferred, not required Must have experience in long term care or skilled nursing facility Reliable transportation required
