As a Hospice Admissions RN at Corus Hospice, you play a pivotal role at one of the most meaningful moments in a patient and family’s journey. Key Responsibilities Conduct comprehensive hospice admissions assessments, including physical, emotional, psychosocial, and spiritual needs Evaluate hospice eligibility using clinical judgment, prognosis, and CMS criteria Educate patients and families on the hospice philosophy, goals of care, and available services Develop and initiate individualized plans of care in collaboration with the interdisciplinary team and attending physicians Coordinate timely admissions by completing accurate documentation and communicating across providers and settings Serve as a clinical liaison with referral sources, facilities, physicians, and internal hospice team members Advocate for patient goals, preferences, and comfort throughout the admission process
