Provide emotional, social, and financial support to patients and families navigating end-of-life care as part of an interdisciplinary team. Responsibilities include conducting social casework, assessing financial needs, and coordinating with community agencies to ensure comprehensive patient care.
Requires a Master's degree in Social Work (MSW) from a CSWE-accredited school and a current state Social Worker license. Candidates must possess a valid driver's license and demonstrate strong clinical judgment and communication skills.
