Provide compassionate personal care and supportive services to patients in their residences to maintain dignity and quality of life. Deliver care in alignment with individualized Plans of Care, focusing on hygiene, medication intake, and mobility.
Must be a Certified Nurse's Aide (CNA) or State Tested Nursing Assistant (STNA) with a valid driver's license and reliable automobile. High school diploma or GED is preferred, along with the ability to work in a multi-disciplinary team setting.

The Hospice Aide is an essential member of the care team and delivers care in alignment with the patient’s individualized Plan of Care.
