About the role

Deliver skilled physical therapy services and implement individualized care plans for patients within their homes. Educate caregivers and patients while completing accurate clinical documentation and supporting care transitions.

  • What You'll Do
  • Deliver skilled physical therapy services to patients in their homes
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Educate patients and caregivers on techniques and home exercises
  • Complete evaluations, progress notes, and discharge summaries in a timely and accurate manner
  • Support care transitions and discharge planning
  • What We're Looking For
  • Licensed to practice as a Physical Therapist in Washington State (or able to obtain before start date)
  • One to two years of experience in physical therapy; home health experience preferred
  • Current CPR certification
  • Valid driver's license, reliable transportation, and auto insurance
  • Strong communication, critical thinking, and time management skills

Requirements
