The ECM Case Manager conducts outreach and engagement to enroll high-risk Medi-Cal members into the Cal AIM program. They develop individualized care plans and coordinate with multidisciplinary teams to link clients to wellness, recovery, and housing resources.
Requires a Bachelor's degree in healthcare or behavioral health (or equivalent experience) and at least one year of case management experience. A valid California driver's license and auto insurance are mandatory for community-based work.
Ability to communicate well with people of diverse cultural professional and experiential background Ability to establish and maintain effective work relationships as part of a multi-disciplinary team. Working conditions This job requires you to be in the community, in client’s homes, schools, social justice offices and other community partner sites as well as work in one or more of our office locations. Driving is essential to the function of this job.
