Manage a caseload of approximately 35 participants by performing assessments and developing Individual Service Plans focused on housing. Provide linkage services to primary care, behavioral health, and other essential community resources while maintaining accurate electronic records.
Requires a combination of education and experience, ranging from a high school diploma with 4 years of experience to a master's degree with 1 year of experience. Candidates must be able to handle psychiatric and medical emergencies and possess strong administrative and communication skills.
Under the supervision of the Site Manager, the Case Manager is responsible for carrying a caseload of approximately 35 participants. The Case Manager performs assessments and develops Individual Service Plans with the participants. The Case Manager will meet with participants to implement Individual Service Plans and collaborate with program leadership to help the organization maintain compliance with local, county, state and federal regulations. PRIMARY DUTIES: Manage a caseload of approximately 35 participants. Perform Individual Service Needs Assessments. Collaborate with the participant to develop an Individual Service Plan that identifies needs with the primary focus being housing. Create an action plan to assist the participant with accomplishing goal(s). Report participant’s progress to Program Leadership as required. Provide case management linkage services and support for housing, income, primary care, substance use treatment, behavioral health, legal, employment, clothing, and other resources. Track and encourage participants to attend scheduled appointments. Maintain accurate participant files and records by entering documentation of all assessments, services, crisis management, and discharge services into various electronic systems for all participants in compliance with ECS practices and contract requirements. Assist participants with planned exits from the program and link to appropriate services. Adhere to agency, state, and funder regulations. Other duties. QualificationsMINIMUM REQUIEREMENTS: Case Manager ($28.00/hr.): High School Diploma + 4 years’ experience or AA degree in similar field + 3 years’ experience, or CHW Certificate/CADC + 2 – 4 years’ experience, or BA/BS degree in similar field + 2 – 4 years’ experience, or MA/MS degree in similar field + 1 year experience. Senior Case Manager ($30/hr.): MA/MS degree + at least 2 years’ experience, or BA/BS degree + at least 4 years’ experience, or CHW Certificate/CADC + 4 years’ experience. PLUS: Associate degree in psychology, counseling, social work, or another related field and 3 years of relevant work experience CCSF Community Health Worker Certificate and 2 – 4 years of relevant work experience High school diploma/GED and 4 years of relevant work experience Experience working in integrated service teams using person-centered harm-reduction and trauma-informed approaches. Background working with unhoused individuals. Effective communication skills (written and oral). Excellent organizational and time management skills. Strong administrative and organizational skills. Must possess excellent charting, recordkeeping, and professional writing skills. Knowledge of Microsoft Office programs and experience documenting services into electronic record systems. Must be able to problem solve and quickly assess/respond to a variety of situations, including medical emergencies, psychiatric emergencies, conflict resolution and de-escalation interventions, as well as other types of crises. Operate with cultural humility in a diverse program and staff population. Possess a knowledge of and sensitivity to adults with substance abuse, domestic violence, incarceration, HIV/AIDS, mental health including co-occurring disorders, and history of trauma. Lived experienced preferred, but not necessary. OTHER: Must secure fingerprint image screening and annual TB screening. Must be able to meet physical requirements of the position, which may include lifting up to 30 lbs.
