Provide comprehensive end-of-life care to patients and their families in their homes, including admissions and ongoing assessments. Responsibilities include medication reviews, coordinating with physicians, and educating caregivers on palliative care.
Requires a nursing degree from an accredited school, a current Minnesota RN license, and AHA BLS certification. Candidates must have at least two years of acute care experience, one year of RN experience, and a valid driver's license with reliable transportation.

include admitting patients to hospice and providing ongoing care for them and their families throughout their hospice journey, medication reviews, seeking orders from physicians, and providing education to caregivers about how to provide end of life care for their loved ones. Some other routine nursing tasks include head to toe assessments, pain assessments, wound care and catheter care. We provide an extensive orientation program as well as ongoing RN office support to you while visiting patients. This position serves in the territory of Sauk Centre, Long Prairie, Clarissa and surrounding areas.
To be eligible for the hiring bonus you must be an external candidate who has not worked for CentraCare for at least 9 months at the date of hire. CentraCare pay promotions to include but not limited to, hiring bonuses may be discontinued at any time due to business needs.
