The RN Case Manager provides clinical oversight, assessments, and care coordination for hospice patients in their homes. They collaborate with physicians to manage and evaluate plans of care while educating patients and their families.
Candidates must be a graduate of an accredited nursing school with a required RN license in Rhode Island and 1-3 years of clinical experience. A BSN and licensure in Massachusetts are preferred.
HopeHealth Hospice & Palliative Care is seeking an experienced RN to care for hospice patients in patient homes in Northern Rhode Island.
The RN Case Manager is an advocate, negotiator, and leader for the clinical oversight of the patient.
The successful candidate is passionate about providing the best possible care at end-of-life and acts with a sense of urgency and a disciplined approach for the accuracy and quality of the care delivered.
30pm hours. There is a rotating weekend commitment of every 3rd-4th weekend.
