The LPN/LVN provides direct patient care and implements established care plans under the supervision of a Registered Nurse. Responsibilities include administering treatments, documenting patient progress, and collaborating with an interdisciplinary team to ensure quality outcomes.
Candidates must be a graduate of an accredited practical or vocational nursing program with a current state LPN/LVN license. At least one year of clinical care experience and valid automobile insurance are required.

The Home Health Licensed Practical/Vocational Nurse (LPN/LVN) is responsible for delivering direct patient care under the supervision of a Registered Nurse. This role involves implementing the established plan of care, administering treatments and medications as prescribed, and supporting patients and families. The LPN/LVN collaborates closely with interdisciplinary team members to promote comfort, dignity, and quality of life, and to ensure that patient care outcomes are met in accordance with home health standards and regulatory guidelines.
Hours • 8: 00 am – 5:00 pm, Monday through Friday
Pay Range $35—$38 USD
Employment is contingent upon the successful completion of a background check. Screening is completed by a third-party administrator, the Michigan Long-Term Care Partnership, and is performed in compliance with the Fair Credit Report Act.
