• Shift: Indirect CareWe believe that the child’s support system is the most valuable blessing. Essential to such support is the appropriate and positive interaction between the child’s caregivers and the staff. Duty Percent of Time Facilitating smooth transitions through the continuum of care from preadmission to post discharge follow up 65% Participates in meetings with the patient/ family and interdisciplinary team 10% Advocates for the patient and hospital for appropriate reimbursement with the insurance company(s), ensuring timely utilization management 5% Informs patient/ family of detailed insurance and benefit information 5% Documents all communication with patient/ family, insurance company and community partners in the electronic health record 10% Other duties as assigned 5%
• Qualifications
• Education: Bachelor degree preferred.

Experience

• 1-2 years of case management experience preferred; 2-3 years of clinical experience with medically complex patients preferred.
• License/Certification/Credential: LPN required; RN preferred. About Us Our work begins where we are all equal - at the heart.
