Requires a graduate of an accredited LPN program with a current Tennessee or compact state license and BLS certification. Candidates should have 1-2 years of LPN experience, preferably in acute care or Med-Surg settings.
The Staff Pad has partnered with a premier hospital in Manchester, TN, to hire experienced an LPN Case Manager for their ER department. In this role, you'll coordinate patient care from admission through discharge, ensuring quality outcomes, seamless transitions, and an exceptional patient experience. The Case Manager collaborates closely with physicians, nurses, and care teams to ensure appropriate level of care, efficient throughput, and safe patient transitions.
Schedule: Full-Time | Days | Nights What You’ll Do
Assess patient needs and develop individualized care plans Conduct utilization reviews to ensure medical necessity and appropriate level-of-care decisions Coordinate admissions, transfers, discharge planning, and community resource referrals Collaborate with physicians, nurses, and interdisciplinary teams to ensure seamless care Identify and resolve barriers to care transitions and patient flow Educate patients and families on treatment plans and follow-up care Monitor patient outcomes and identify opportunities for improvement Ensure compliance with hospital policies, regulatory standards, documentation and payer requirements Advocate for patients throughout their healthcare journey
Graduate of an accredited Practical Nursing (LPN) program Current LPN license (Tennessee or compact state) BLS certification required, ACLS preferred Minimum 1-2 years of LPN experience, preferably in an ER/acute care or Med-Surg setting Previous case management or utilization review experience preferred Strong clinical judgement, communication, critical-thinking, and organizational skills Knowledge of care coordination, utilization review, and discharge planning Excellent teamwork abilities and able to thrive in a fast-paced high-pressure environment Why Join This Opportunity?
