Responsible for collecting and reporting statistical information regarding births, stillbirths, and deaths to complete legal birth certificates. The role involves researching medical records, interviewing patients, and notarizing paternity affidavits.
Requires a High School Diploma or GED and the ability to obtain a Notary commission. Candidates must have one year of clerical experience and a typing speed of 35 wpm.
Gathers information through researching medical record, Epic and patient interview in order to complete the legal birth certificate. Completes the legal birth certificate and submits within the specified guidelines established by state law for retention by the Bureau of Vital Statistics. Completes and notarizes paternity affidavits to assist unmarried parents establish paternity within the Commonwealth of Kentucky. Provides verification of application for social security numbers to mothers to assist them with their administrative needs. Enters baby name in Epic to assist physicians and patient needs. Performs other duties as assigned.
