Deliver interactive, community-based mental health services to adults and children to enhance overall functioning in the least restrictive environment. Coordinate healthcare services and facilitate communication between clients, families, and community partners.
Requires a Bachelor's degree or an Associate's degree with additional experience, totaling at least 4 years of combined education and experience. Must possess CPST Case Management training and pass various background checks and registries.
Required Education and Experience One of the following is required: Bachelor’s degree, preferably in Social Work, Psychology, or some related field Associate's degree with an additional 2 or more years' experience working with educating and teaching clients about self-care, improving daily living activities Total combined years of schooling and experience (working with educating and teaching clients about self-care, improving daily living activities) to equal 4 or more years Required License/Certifications/Registrations CPST Case Management training, KHS training, pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens. Completion of Adult Case Management training within 60 days of employment. Valid driver’s license, insurance, and a dependable vehicle if position requires driving.
Preferred Education and Experience Bachelor’s degree in Social Work, Sociology, Psychology, or Special Education (with specialized training in human development and family dynamics). Three to five years’ professional experience in mental health services, crisis response, or a related field. Preferred License/Certifications/Registrations none Knowledge, Skills, and Abilities: Familiarity with community mental health services. Strong organizational skills Intermediate proficiency with Microsoft Office Physical Requirements: With or without accommodation. Light Work: Occasionally exerting up to 25 lbs - frequently exerting up to 10 lbs. 11-25% of the day may be standing or walking.
