Position Summary
Our dedicated Continuing Care nursing team make a lasting difference in the lives of patients with complex, long-term medical needs. Here, you will build meaningful relationships and provide compassionate, holistic care that promotes independence and quality of life. Key Responsibilities: Provide compassionate, evidence-based nursing care to patients in a long-term acute care or rehabilitation setting. Collaborate with physicians, therapists, and other care team members to develop and implement individualized care plans. Monitor, assess, and document patient progress while ensuring safety and comfort. Educate patients and families on treatment plans, medication management, and discharge planning. Support a culture of excellence, teamwork, and continuous learning.

Required: Current Registered Nurse (RN) license in the State of Connecticut.
