OSF HealthCare Neuroscience Institute | Peoria, Illinois OSF HealthCare Neuroscience Institute is seeking an Advanced Practice Provider to join a collaborative, team-oriented group of APPs and Neurologists providing comprehensive care to the general neurology population in beautiful Peoria, Illinois. Our outpatient neurology practice includes 14 neurologists, with 3 general neurologists and 4 APPs dedicated to General Neurology. We are a comprehensive neuroscience program strongly committed to the education, training, and mentorship of Advanced Practice Providers, with structured onboarding, specialty support, and opportunities for professional growth.
Schedule: Monday–Friday Option of four 10 1/2-hour shifts or five 8 1/2-hour shifts Practice setting: Hospital Outpatient Department Focus: Evaluation and longitudinal management of general neurology patients Outpatient APP Responsibilities The Advanced Practice Provider functions as an integral member of the neurology care team and provides comprehensive outpatient care, including: Obtain detailed medical histories Perform neurological and physical examinations Conduct comprehensive assessments Order, perform, and interpret diagnostic studies Formulate differential diagnoses and diagnostic impressions Develop and implement evidence-based treatment plans Monitor response to therapies and adjust care accordingly Prescribe pharmacologic and non-pharmacologic treatments Coordinate consultations and referrals Provide patient and family counseling and education Document care in accordance with regulatory and quality standards Practice Model APPs work collaboratively with general neurologists and subspecialists across the Neuroscience Institute to manage a broad spectrum of neurological conditions, including but not limited to: Stroke and cerebrovascular disease follow-up Epilepsy and seizure disorders Headache and migraine disorders Movement disorders Dementia and cognitive disorders Multiple sclerosis and demyelinating disease Peripheral neuropathies General neurological complaints The practice emphasizes team-based care, timely access, and continuity across the outpatient neuroscience continuum. Core APP Responsibilities Clinical Evaluation Perform comprehensive neurological assessments, identify red-flag symptoms, initiate appropriate diagnostic workups, and collaborate with neurologists for complex presentations. Diagnostics and Care Coordination Order and interpret diagnostic studies, reconcile results, coordinate specialty referrals, and ensure seamless transitions across services. Medication and Treatment Management Initiate, titrate, and monitor neurological therapies. Manage side effects, provide patient education, and promote adherence to treatment plans. Symptom and Supportive Care Address pain, fatigue, cognitive concerns, mood symptoms, functional limitations, and quality-of-life needs through multidisciplinary collaboration. Interprofessional and Operational Roles Independently manage follow-up visits for stable general neurology patients, support urgent access needs, assist with quality and clinical initiatives, and actively participate in team-based clinic operations. Education and Care Transitions Provide disease education, lifestyle counseling, and safety guidance. Support care transitions, including referrals to therapy services, community resources, and specialty programs as indicated. Additional compensation components may be applicable such as bonuses or incentives which may or may not be based on metric achievement.
