Job Description
Job Summary Responsible for providing comprehensive patient care and treatment under physician supervision and in accordance with established regulations, privileges, and protocols. Delivers health promotion, disease prevention, and management of acute and chronic conditions for patients of all ages. Ensures accurate assessment, diagnosis, and implementation of care plans. Collaborates with interdisciplinary teams to optimize patient outcomes. Maintains compliance with clinical standards and organizational policies. Essential Functions
Basic understanding about overall CKD care, hypertensive and diabetic kidney disease; should have understanding when to suspect Glomerular disease, some basic information about cystic kidney disease, kidney stones, Electrolyte and Mineral abnormalities
Strong understanding of renal pathophysiology, pharmacology and dialysis modalities to have a good understanding and provide both Medial and Dialysis treatment need from kidney standpoint. (Typically for evaluation and treatment for acute and chronic kidney dysfunction, causes related to it, electrolyte and acid base disorder, minerals, kidney related hormone and vitamin related disorder) Provides health promotion, disease prevention, and management of acute and chronic conditions for patients of all ages.
Competence in taking care of different types of dialysis treatment orders for HD (hemodialysis), PIRRT (prolonged intermittent RRT), SLED (slow low effusion dialysis), CRRT (continuous renal replacement therapy), PD (peritoneal dialysis).
General knowledge of common kidney protective medications, Blood Pressure medications, Diuretic medications, Electrolytes and Minerals related medications, Anemia of chronic kidney disease related medication. Basic understanding about Stones or cystic kidney disease or Kidney mass related medications and Transplant related medications.
Treats patients diligently and skillfully in accordance with current standards, laws, and regulations.
Consults with medical staff regarding cases beyond professional scope or ability.
Prescribes medications and provides medication-assisted treatment within scope of practice.
Delivers behavioral health services to outpatients as indicated by practice needs.
Participates in ongoing patient management activities, including medication and schedule management, pharmacy coordination, and quality initiatives.
Performs and documents patient care activities, including histories, physicals, discharge summaries, service plans, progress notes, and informed consent.
Assists in training and educating students, nurses, and paramedical personnel in relevant specialty areas.
Maintains accurate, timely, and compliant medical records and documentation.
Performs other duties as assigned.
