The Nurse Practitioner (NP) in Pulmonary and Critical Care provides comprehensive outpatient care for patients with Interstitial Lung Disease (ILD) and other complex pulmonary conditions.
Job Description: Primary
Provide comprehensive outpatient care for patients with interstitial lung disease (ILD) and other complex pulmonary conditions. Perform advanced diagnostic evaluations and assessments for patients with suspected or established ILD. Manage high-acuity patients with progressive respiratory disease, significant symptom burden, multiple comorbidities, and frequent hospitalizations. Develop, implement, and monitor individualized treatment plans, including immunosuppressive therapies, antifibrotic medications, and oxygen management strategies. Conduct longitudinal disease monitoring and adjust treatment plans based on clinical status, diagnostic findings, and patient response. Coordinate care across multiple specialties to ensure seamless, patient-centered management. Collaborate closely with a multidisciplinary ILD team, including physicians, nurse coordinators, and other healthcare professionals. Provide patient and family education regarding disease processes, treatment options, medication management, and self-care strategies. Participate in care transitions and hospitalization follow-up to reduce readmissions and optimize patient outcomes. Document patient encounters and treatment plans in accordance with organizational, regulatory, and quality standards. Engage in ongoing ILD-specific training and professional development to maintain expertise in the diagnosis and management of interstitial lung disease.
