Provide wraparound case management services to youth and young adults with serious emotional disturbances in southeast Nebraska. Coordinate with schools and community providers to develop individualized, strength-based plans that prevent out-of-home placements and support successful transitions to adult services.
Requires a bachelor's degree in human services or a related field, or 3+ years of equivalent experience. Candidates must have reliable personal transportation, a satisfactory background check, and proficiency with remote work tools.
Work independently from home while serving clients in the community.
