About the Role

• The Licensed Practical Nurse (LPN) Weekend Option at CarDon and Associates plays a critical role in delivering high-quality nursing care during weekend shifts.
• This position focuses on providing direct resident care, ensuring resident comfort, and supporting the healthcare team in managing resident needs effectively.
• The Licensed Practical Nurse (LPN) Weekend Option will be responsible for monitoring resident conditions, administering medications, and performing wound care with precision and compassion.
• Additionally, the role involves educating residents and their families about health management and advocating for residents' rights and well-being.
• Ultimately, the LPN contributes to a safe, supportive, and healing environment that promotes positive health outcomes for all residents.

• Job Responsibilities
• Provide direct resident care including medication administration, wound care, and telemetry monitoring during weekend shifts.
• Assess and document patient conditions, reporting any changes promptly to the healthcare team.
• Educate residents and their families on health maintenance, treatment plans, and post-discharge care instructions.
• Advocate for residents needs and preferences, ensuring their rights and dignity are respected at all times.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.

Minimum Qualifications

• Current and valid Licensed Practical Nurse (LPN) license in the state of Indiana.
• Basic Life Support (BLS) certification.
• Experience in direct resident care, including medication administration and wound care.
• Ability to work independently and as part of a healthcare team during weekend shifts.

Preferred Qualifications

• Previous work experience in a long-term care or rehabilitation setting.
• Strong communication and resident advocacy skills.
• Familiarity with electronic health record (EHR) systems.
