Position Summary

We are seeking a compassionate and highly organized Licensed Practical Nurse (LPN) to provide one-on-one care in a home setting in West Bloomfield. This role requires strong clinical skills, attention to detail, and the ability to support both medical and daily living needs while helping the client maintain an active and engaged lifestyle.

Schedule

• Week 1:
• Tuesday,
• Wednesday and
• Friday 8 a - 8 p
• Week 2:
• Tuesday,
• Saturday,
• Sunday 8 a - 8 p
• Key
• Responsibilities: Provide skilled nursing care in accordance with the care plan Perform tube feedings and monitor nutritional intake Assist with and guide range of motion exercises Monitor and report changes in client condition Maintain accurate and timely documentation Coordinate and manage doctor appointments and transportation logistics Support scheduling and participation in social and recreational activities Communicate effectively with family members and the care team

Qualifications
