As a Home Health Registered Nurse, you'll provide skilled, compassionate care that helps patients recover, manage chronic conditions, and maintain independence in the comfort of their own homes. You'll build meaningful relationships with patients and families while enjoying the autonomy to manage your schedule and make a direct impact on patient outcomes.

• What you'll do
• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care and ongoing patient evaluations.
• Educate patients and caregivers on disease management, medications, nutrition, and self-care.
• Collaborate with physicians and interdisciplinary team members to coordinate care.
• Monitor patient progress and adjust care plans as needed.
• Document clinical findings and maintain compliance with home health regulations.
• Supervise and support Home Health Aides and LVN/LPN team members.
• Promote patient safety, infection prevention, and successful recovery at home.

Qualifications

• Current RN license in the state of practice
• Valid driver's license and auto insurance
• Current CPR certification
• Home Health experience preferred
• HomeCare HomeBase experience preferred
