The Occupational Therapist is responsible for conducting screenings and evaluations to develop and implement individualized treatment plans for adult and geriatric patients. They must monitor patient progress, maintain accurate records, and supervise occupational therapy assistants.
Candidates must have successfully completed an accredited Occupational Therapy program and hold a current valid license to practice in the state. Strong communication, critical thinking, and a compassionate approach to patient care are required.

Are you passionate about enhancing lives? We are seeking an Occupational Therapist to join our team at Meadows on Fairview Care Center in Wyoming, MN!
As an Occupational Therapist, you will be responsible for directly participating in occupational therapy treatment interventions and services, in accordance with state practice guidelines. This position provides direct patient care to primarily adult and geriatric patients.
Provides screenings and evaluations to determine requirements for skilled occupational therapy intervention relative to patient’s prior level of function. Develops and implements treatment plans that focus on functional and measurable outcomes and adheres to professional standards of practice. Monitor patients' progress and adjust treatment plans as needed. Provides appropriate supervision for occupational therapy assistants in compliance with licensure, regulatory and company requirements. Maintain accurate and up-to-date patient records. Collaborate with, coordinate, and participate in training programs for community staff, patients, families, and rehabilitation staff. Participate in continuous quality improvement activities.
Education and Experience: Successful completion of an accredited Occupational Therapy program. Knowledge, Skills, and Abilities: Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Strong problem-solving and critical thinking skills. Compassionate and patient-centered approach to care. License and Certification: Current valid Occupational Therapist license to practice in appropriate state.
