Responsibilities

• Administer prescribed medications and treatments per the plan of care and facility policies.
• Monitor residents’ health status, document vital signs, and escalate changes promptly to the care team.
• Perform wound care, routine procedures, and assist with peri-care and injections as ordered.
• Assist residents with activities of daily living and mobility; support safe transfers and use of assistive devices.
• Collaborate with the interdisciplinary team; communicate care plans and progress to colleagues and families as appropriate.
• Maintain accurate documentation in the electronic health record and ensure compliance with infection control and safety protocols.

Requirements

• Active Licensed Practical Nurse license (state-issued; eligible to practice in the assignment state)
• Current CPR/BLS certification
• Ability to work 12-hour shifts in a long-term care setting
• Strong communication skills and ability to collaborate with an interdisciplinary team
• Prior long-term care experience or familiarity with LTC care plans preferred
• Ability to meet health, safety, and vaccination requirements as required for the assignment
