Responsibilities

• Conduct psychosocial assessments for adults with substance use disorders and dual diagnoses; develop and implement treatment plans.
• Provide counseling, case management, discharge planning, and referrals to community resources; document in EPIC EMR.
• Collaborate with a multidisciplinary team to coordinate care and support transitions across settings.
• Monitor patient progress, identify barriers to care, and advocate for continuity of services and supports.
• Ensure compliance with regulatory requirements, complete required documentation, and participate in required orientations.

Requirements

• MA Licensed Social Worker (LCSW) in good standing; LCSW required; CANS certification required at time of submittal.
• Master’s degree in Social Work (MSW) from an accredited program.
• AHA BLS certification required.
• Proficiency with EPIC EMR preferred; experience with substance use disorders and dual diagnosis is highly preferred.
• Local candidates within 50 miles eligible for local rate; must be MA LCSW; must document travel experience; RTO requests must be included at submission and are not guaranteed after confirmation.
• Flu vaccine required during flu season; ability to work in a daytime, ambulatory clinic setting; 2 recent Manager/Supervisor references; 2+ years of social work experience and travel experience documented.
