Responsibilities

• Conduct initial and ongoing functional assessments of home-based patients and develop individualized PT plans of care.
• Implement evidence-based PT interventions to improve mobility, strength, balance, gait, and pain management.
• Regularly reassess patient progress and modify the plan of care to meet goals and optimize outcomes.
• Educate patients and caregivers on exercises, activity modification, safety, and use of assistive devices; document home exercise programs.
• Maintain accurate documentation in the electronic health record and communicate findings with the supervising PT and the care team.
• Ensure patient safety and infection control in home environments and support appropriate discharge planning.

Requirements

• Active California physical therapist license
• Basic Life Support (BLS) certification required
• TB clearance completed within the previous 12 months
• Ability to work in a home health care setting and travel to patient homes as needed
