REGISTERED NURSE (RN) - Home Health SummitWest Care | Mesa County, CO Full-Time | Monday-Friday Day Schedule + On-Call Rotation $35–$43/hour (DOE) + Mileage Reimbursement A Sustainable Approach to Nursing SummitWest Care is a nonprofit home health agency seeking a Registered Nurse (RN) to provide skilled, patient-centered care in the community.
This is an in-person home health position based in Mesa County, CO, offering flexibility in how you structure your day, along with manageable caseloads and meaningful one-on-one care.
What We're Looking For Active Registered Nurse (RN) License in State of Colorado (or active NLC compact license) Valid driver's license, reliable transportation, and auto insurance Strong assessment and critical thinking skills Ability to work independently and manage a structured caseload Clear, professional communication with patients and team members Comfort with documentation and EMR systems (HCHB preferred) Home health experience preferred, but not required What You'll Do Perform comprehensive patient assessments Develop and update individualized plans of care Provide skilled nursing care and appropriate interventions Monitor patient progress and report changes to the care team Educate patients and families on medications, treatments, and care plans Coordinate care with LPNs, therapists, caregivers, and providers Complete, timely, and accurate documentation in EMR Participate in on-call rotation Work Environment Home health offers a different pace and structure than facility-based care.
