Provide care coordination through the Healthy Start Program to ensure eligible pregnant women have access to prenatal and post-partum medical care. Connect families with social services to remove barriers to care and teach basic maternal and baby care to ensure healthy births.
Requires a high school diploma or equivalent and a valid Ohio driver's license with automobile insurance. Candidates must be culturally sensitive, possess strong communication skills, and have knowledge of community resources.
THIS IS A COLLECTIVE BARGAINING UNIT POSITION Provide care coordination through the Healthy Start Program for program-eligible pregnant women to help ensure these women have a medical home for prenatal and post-partum care, practice appropriate pregnancy self-care, get connected to needed social services to remove barriers to care, and learn basic maternal and baby care with the goal of achieving a full term, healthy birth weight birth. EDUCATION Requires a high school diploma or equivalent and knowledge of community resources. Prefer a Certified Community Health Worker.
Must have a valid Ohio driver’s license and automobile insurance. Must be able to share educational information and resources with individuals and families. Must be culturally sensitive to families and demonstrate respect in all interactions. Must maintain confidentiality at all times. Must have good communication skills.
