The Case Manager provides direct casework services, including conducting psychosocial assessments and developing individualized service and treatment plans for clients. They are responsible for managing a caseload, making resource referrals, and tracking client progress through detailed documentation.
A Bachelor's degree in Social Work or a related field and certification in First Aid and CPR are required. Preferred candidates have two years of experience working with veterans, MICA, or homeless populations.

Essential job functions: Through the employee’s own efforts, the employee accomplishes the following essential functions:

To perform this job successfully, and individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and / or ability required.
Ascending or descending stairs and ramps using feet and legs and/or hands and arms. Must be able to walk up and down stairs (about 10 flights) in order to patrol stairways, respond to security incidents, and assist in the evacuation of clients during an emergency. Body agility is emphasized. The amount of climbing required exceeds that required for ordinary locomotion.
