Provide therapeutic emotional support, education, and resource information to patients and families facing serious illnesses. Advocate for patient needs and execute the Plan of Care in coordination with the healthcare team.
Requires a Bachelor of Social Work with at least one year of experience and a valid Texas State license. A Master's degree and hospice or healthcare experience are preferred.
