Perform comprehensive patient assessments and develop individualized care plans for patients in their homes. Collaborate with physicians and supervise Home Health Aides and LPN/LVNs to ensure high-quality outcomes.
Must possess a current RN license, valid driver's license, CPR certification, and auto insurance. Preferred candidates have 2+ years of home health experience and familiarity with HomeCare HomeBase.
