Develop individualized stabilization plans for Veterans focusing on housing, employment, and wellness needs. Manage a full caseload through direct client engagement, community outreach, and compliance-driven documentation.
Requires 1-3 years of experience in case management or social services and a valid driver's license with reliable transportation. An Associate or Bachelor's degree in Social Work or a related field is preferred.
Experience with homelessness prevention, rapid re-housing, housing stabilization, SSVF, VA programs, or transitional support services. Experience using HMIS or similar case management/data systems. Military or Veteran experience is not required but is strongly valued. Demonstrated passion for serving Veterans and helping remove barriers to housing and stability. Strong analytical skills, flexibility, and adaptability in changing environments. Compensation The salary range for this position is $55,000–$58,000 annually. The final offer will be based on experience, qualifications, and alignment with program funding.
