About the Role

Matter Health is seeking an Advanced Practice Provider/Nurse Practitioner to support older adults across Chattanooga, TN. This role may be full-time or part-time depending on market needs.

At Matter Health, our APPs are more than providers — they are Panel Owners. You will manage a dedicated panel of older adults, serving as their longitudinal primary care provider while building trusted relationships directly within the communities you serve.

This is a fully community-based role with no traditional clinic home base. APPs operate with a high level of clinical autonomy while partnering closely with an interdisciplinary care team and dedicated Medical Director for collaboration and support with complex cases. What You'll Do
Provide longitudinal primary and preventive care for older adult patient populations
Manage and grow a dedicated patient panel within assigned communities
Deliver patient-centered, relationship-driven care directly in community settings
Collaborate closely with interdisciplinary clinical and operational teams
Support preventative care, chronic disease management, and care coordination initiatives
Utilize telehealth and in-person care delivery models to improve healthcare accessibility
Maintain accurate and timely EHR documentation and coding specificity

Qualifications

Active, unrestricted Nurse Practitioner or Physician Associate license
Board Certification: NP (AANPCB or ANCC — family, adult, geriatric, or adult-gerontology) PA (NCCPA)
Active DEA license and controlled substance registration where applicable
3+ years of clinical experience as an APP, preferably with aging or medically complex populations
Basic Life Support (BLS) certification
Valid driver’s license and reliable transportation (car stipend provided)
Experience with EHR documentation (athenahealth experience preferred)
Commitment to equitable, patient-centered care Strongly Preferred
Background in family medicine, internal medicine, adult medicine, or geriatrics
Experience in value-based care environments
