Advanced Practice Provider/Nurse Practitioner

Matter Health is seeking an Advanced Practice Provider/Nurse Practitioner to support older adults across Springfield, MA. This role may be full-time or part-time depending on market needs.

At Matter Health, our APPs are more than providers — they are Panel Owners. You will manage a dedicated panel of older adults, serving as their longitudinal primary care provider while building trusted relationships directly within the communities you serve.

• This is a fully community-based role with no traditional clinic home base. APPs operate with a high level of clinical autonomy while partnering closely with an interdisciplinary care team and dedicated Medical Director for collaboration and support with complex cases. What You'll Do
• Provide longitudinal primary and preventive care for older adult patient populations
• Manage and grow a dedicated patient panel within assigned communities
• Deliver patient-centered, relationship-driven care directly in community settings
• Collaborate closely with interdisciplinary clinical and operational teams
• Support preventative care, chronic disease management, and care coordination initiatives
• Utilize telehealth and in-person care delivery models to improve healthcare accessibility
• Maintain accurate and timely EHR documentation and coding specificity

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Associate license
• Board Certification: NP (AANPCB or ANCC — family, adult, geriatric, or adult-gerontology) PA (NCCPA)
• Active DEA license and controlled substance registration where applicable
• 3+ years of clinical experience as an APP, preferably with aging or medically complex populations
• Basic Life Support (BLS) certification
• Valid driver’s license and reliable transportation (car stipend provided)
• Experience with EHR documentation (athenahealth experience preferred)
• Commitment to equitable, patient-centered care Strongly Preferred
• Background in family medicine, internal medicine, adult medicine, or geriatrics
• Experience in value-based care environments
• Experience providing care in community-based, in-home, or non-traditional healthcare settings
