This position requires you to work on site at one of our facilities located throughout Broward County This is an OPS (Other Personal Services) positions and as such no paid time off is offered. The functions of the Health Services Representative ( HSR) is to receive incoming reports of Sexual Transmitted Infections from health care providers and laboratories, to process the reports byentering health data into the Patient Reporting Surveillance Manager (STARS) Database and to interact with external health care providers and laboratories and obtain additional health information forcase management purposes. This position reports to the Sexually Transmitted Diseases (STD) ProgramSurveillance Supervisor. Works in the STD Program, HSR ensures all STD records are entered in a timely manner into STARS Database. In addition, files and reviews medical records and Field Records (FR) to verify patienthistory, diagnosis, treatment and other pertaining information to assist the Disease InterventionSpecialist (DIS) and Supervisor. Interacts professionally with staff, health care providers and laboratory personnel to collect patient demographics and medical confidential information. Acts as liaison between the STD Program and other County Health Department Disease Control Programs to provide and elicit appropriate patient information and demographics updates. Serves a “back-up” for other HSR’s in the STD Program. Attends staff meetings, trainings and staff development activities as required. Valid driver’s license, free of major infractions and access to an automobile to be utilized for work purpose. Experience working in the STD or HIV field. Able to work nights and weekends according to program needs. Willing to participate in STD community outreach. Intermediate level (or higher) computer skills.
Minimum of one (1) year of experience working in the STD or HIV field. Associate degree or Bachelor’s degree in Public Health or a related field from an accredited college or university. One (1) or more year's experience with case management, or public health. Bilingual fluent in English/Spanish
