Provide housing case management and support for adults transitioning from correctional facilities or psychiatric centers into the community. Responsibilities include locating affordable housing, assisting with benefits, and maintaining detailed progress notes in the Cerner system.
Requires a high school diploma with four years of experience, an associate degree with two years, or a bachelor's degree with one year of experience working with mental health recipients. A valid driver's license and a working motor vehicle are mandatory.

To Provide housing case management services and support to adult individuals being released from Correctional Facilities directly into the Community and/or from New York State Psychiatric Centers into the community. These individuals are identified as having serious and persistent mental health issues and are often linked with NYS Parole.
