PRN Pharmacist

This is a PRN Pharmacist position that will be scheduled as needed. This position will be located in MLIH's pharmacy at our 3 Mile location in Logan, WV. Will require some Saturday work in the future.

Position purpose

This position is responsible for aiding customers by compounding and/or dispensing prescription medications, providing pharmaceutical information to healthcare professionals, monitoring customers’ drug therapies to avoid interactions with other medications, and providing pharmaceutical expertise to customers on the safe use of medications.

Essential job functions

• 1. Reviews prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability. Provides information and advice regarding drug interactions, side effects, dosage, and proper medication storage. 2. Assesses the identity, strength, or purity of medications. Compounds and dispense medications as prescribed by doctors and dentists by calculating, weighing, measuring, and mixing ingredients, or overseeing these activities. 3. Maintains records such as pharmacy files, patient profiles, charge system files, inventories, control records for radioactive nuclei, and registries of poisons, narcotics, and controlled drugs. 4. Maintains procedures for mixing, packaging, and labeling pharmaceuticals according to policy and legal requirements to ensure quality, security, and proper disposal. 5. Provides specialized services to help patients manage conditions such as diabetes, asthma, smoking cessation, and high blood pressure. Offers health promotion and prevention activities by, for example, training people to use devices such as blood pressure and diabetes monitors. Advises customers on the selection of medication brands, medical equipment, and health care supplies. 6. Collaborates with other health care professionals to plan, monitor, review, and evaluate the quality and effectiveness of drugs and drug regimens, providing advice on drug applications and characteristics. Analyzes prescribing trends to monitor patient compliance and to prevent excessive usage or harmful interactions. 7. Orders and purchases pharmaceutical supplies, medical supplies, and drugs, maintaining stock and storing and handling it properly. 8. Other duties as assigned by immediate supervisor. 9. Pharmacists have an important role in Patient Centered Medical Home (PCMH) and care of patients with chronic medical conditions, including, by performing the following duties (including, but not limited to):
• pharmaceutical management
• medication counseling with patients
• serving as a clinical resource for providers
• working on practice teams with providers and ancillary staff.

Minimum qualifications

Graduation from an accredited school of Pharmacy, with a Doctor of Pharmacy (Pharm. D.) degree. Two years of experience in the field, preferred.
