Identify patients who are vulnerable, high risk for readmission within 30 days of discharge, and/or high ED utilizers requiring additional support and connections to community resources.
Collaborate with patients, ED staff, ED physicians, and community providers to develop a comprehensive plan of care for safe discharge. Completes required documentation in a timely and accurate manner
Act as a patient advocate by identifying and connecting patients to the most appropriate community resources. Maintain a directory of available services by geographical area and update others about new services. Consult with others to identify potential community resources for resolving client health, psychosocial, or financial barriers.
Collaborate with behavioral health team and other community groups to ensure patients are connected to the most appropriate mental health programs in the community.
Identify patients with admitting orders still in the ED. ED Care Navigator will interview and complete assessment on the patients / family members to formulate a plan of care prior to transferring to a unit.
Collaborate with ED interdisciplinary team to create CRISP Care Alerts for complex and high utilizing patients Qualifications include:
