Provide psychosocial-emotional support and comprehensive assessments to patients and families dealing with illness. Collaborate with multidisciplinary teams to implement patient-centered treatment plans and manage care.
A Master's Degree in Social Work and at least one year of clinical or case management experience are required. Candidates must hold a current Basic Life Support certification from the American Heart Association.
Responsible for providing psychosocial-emotional support and services to patients and families. Assists patient and family in understanding, coping and adapting to the personal, emotional and bio-psychosocial issues associated to the patient's illness. Provides comprehensive assessments and referrals, crisis intervention and care management. Collaborates with multi-disciplinary team and other internal and external personnel to promote an awareness of environmental and psychosocial needs and to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community. May provide functional direction to unlicensed social work staff and social work interns.
